Prehabilitation referral

If the patient is undergoing treatment for cancer, needs non-cancer surgery or is awaiting a liver transplant either currently or in the near future, you can refer them to prehabilitation.

Please choose the type of referral and fill in the corresponding referral form as thoroughly as possible. This will help us to speed up the process and get in touch with the patient as soon as possible.
